CHC Software Solutions

As a user, the CHC software may initially collect the following information from you:

including potentially sensitive, personally identifying information and personal health information (personal information).  This can include, but is not limited to, your date of birth, diagnosis, appointment dates, health history, lab results, session documentation, and claim detail. This information will be available within the software solution system to the authorized users based on the subscriber’s access controls policy.  The use and disclosure of this personal information is governed by the subscriber’s privacy practices. For information regarding such privacy practices, please contact a subscriber representative and request a copy of their privacy practices.  Your subscriber representative is generally the business entity who has introduced and offered the CHC software product to you. CHC does not separately retain or record information provided by you during any telepresence session.

Help Desk and Solutions Portals

As a user CHC may collect the following information provided by you:

This information is necessary to register an individual as an authorized user to the subscriber’s software solution product. The system also uses this information to confirm a user’s identity and sync to an internal record within the subscriber’s software portal. Furthermore, collection of data allows users to make use of the functionality of the software to interact with the subscriber representative.

Help Desk and Solutions Portals

This information will be used primarily to help troubleshoot issues experienced with the software and/or service by the subscribers.  CHC may use the information for improvements that will serve to make the experience for all users more pleasant. This also serves as a sounding board for peer to peer experiences using any CHC Software Solutions.

CHC is not responsible for your handling, sharing, re-sharing and/or distribution of personal information stored within the software. CHC does not block individuals under the age of 18 from submitting information via the CHC website. Submitted information will be reviewed carefully and authenticated by authorized personnel before response is provided.

CHC Software Solutions

Because users of our systems may be under the age of 18, CHC does not prohibit access by children. At time of claim/case creation, the subscriber’s authenticated user is responsible for validation of identity and age, as well as any necessary guardian involvement and documentation.

Your subscriber representative is generally the business entity who has introduced and offered the CHC software product to you.
